To register your child for any of the Eagle Sports teams, please download the

2013-2014 Eagle Sports Registration Form (PDF format)
2013-2014 Policies and Information (PDF format)

Print out both forms, fill out the required information, and sign. Send both pages of the signed registration form and the last page of the policies document to:

Fees cover costs for facilities, referees/umpires, uniforms, etc. Projected participation fees for all sports are listed below. A $25 registraion fee is required per athlete per year. (Non-refundable) If you would like us to add a sport or activity, let us know so we will consider it in the near future.

A $25 registration fee is due if the athlete had not participated in an earlier sport.
